SU faculty and staff interested in applying for the Syracuse University Medical Aid Program (SUMAP) must submit the application, a copy of the federal tax return from 2008 and W-2 forms for the employee and his or her spouse/same-sex domestic partner from 2008 to HR by Tuesday, December 1. Applications, together with the required documentation, must be received in the office by 5 p.m.

Applications that are submitted with the required documentation after the deadline will be reviewed and placed on a waiting list if they meet eligibility requirements.
