Medical Plan Assistance (SUMAP) information session to be held

The Office of Human Resources is holding informational sessions for SU faculty and staff interested in applying for the Syracuse University Medical Aid Program (SUMAP), a program that provides financial assistance with medical plan costs.

Eligibility for SUMAP is based on household taxable income and household size (number of dependents), relative to the Federal Poverty Level. Faculty and staff who qualify for SUMAP will receive assistance in one of two ways:

• Group 1 Benefits—Qualified faculty and staff are expected to receive a 75% reduction in their SU medical plan out-of-pocket maximum and a 25% subsidy to their monthly plan contributions.

• Group 2 Benefits—Qualified faculty and staff are expected to receive a 75% reduction in their SU medical plan out-of-pocket maximum.

The last informational session about SUMAP will be held:

Tuesday, Nov. 24, at noon in 310 Lyman Hall.

All sessions are open to all faculty and staff, and no R.S.V.P. is needed. There will be HR staff on hand at these sessions to provide information and assistance with completing the application, which is due Dec. 1.

To apply for the program, you must submit the application, a copy of the federal tax return and W-2 forms filed for 2008 by you and your spouse/same-sex domestic partner from 2008 to HR by Tuesday, Dec. 1. Applications that are received with the required documentation by 5 p.m. on Dec. 1 will be reviewed for eligibility in SUMAP in 2010.
